  After School Club Parental Agreement

  Ampertaine Primary School Extended Schools Services are school-based services provided and maintained by Ampertaine Primary School.
  
  They are a range of wrap-around care services aimed at relieving the financial pressures faced by many families in the current climate.
  
  The purposes of the Extended Schools Services are to provide a community service. Any profit made is re-invested back into the services or used to directly improve the educational provision of the school. As such, the small fees involved require very careful financial management.
  
  After School Club operates every school day with exception of half or part-days. Service will not operate on exceptional closures or holidays.
  
  1 session daily from 2:00pm – 5:00pm
  
  Pupils can be booked in advance as often as required.
  
  A paper copy can be obtained from the school office on request.

  Medicine Forms 26 27

  We ask ALL parents and guardians to complete the form to aid our staff in helping our pupils. Forms can be downloaded from the school website or requested from the school office.
  
  A copy of this form must be sent to school every time a child needs to take medication during school hours.
  
  The following information is required.
  Child’s Name, Class, Date, Name of medicine, Dose required, the Exact Time(s) and parental/guardian signature.
  
  Please make sure that you understand and have followed the following guidelines exactly:
  
  The minimum dose has been sent to school.
  The medicine is in its original, labelled container.
  I understand that teachers may refuse to assist with medicines.

  School Flu Vaccination Programme

  The flu vaccination programme in schools will be going ahead in the autumn term. It is likely that flu and other respiratory infections will be circulating at the same time which means that it is important to help protect children against flu and prevent the spread of flu in the community.

  Tempest Photos – Tuesday 8th September

  Photography will begin at 8:30am for those wishing to have a family photo involving children who do not presently attend Ampertaine Primary school.
  
  Please inform either school or little amps before Tuesday 8th if you wish to have a group photo taken
  Payments will be made to Tempest and like last year these will be made online via the Tempest website.
  
  All pupils will have individual photos taken allowing parents the option to purchase or not.

  FREE Breakfast Club (8 – 8:40am) All pupils MUST be in school by 8:20am

  Last school year we had a fantastic amount of pupils attending our club each morning and look forward to welcoming everyone from Tuesday 1st September.
  
  Thank you also to those who have donated food already for our club. We are gladly accepting the following items
  • Apple Juice
  • Orange Juice
  • Money! Last year our club cost on average £18 a week.
  
  Pupils are offered juice (apple or orange), toast and a choice of cereal. We are currently receiving free cereal from a community group.
  
  The Club is a great opportunity for our pupils to mix and socialise with each other before they start their busy day.
  
  Thank you so much to our parental helpers. Our club would not be operational without the parental support each morning.
